FALL WATER POLO - IMPORTANT INFORMATION

Parents and Athletes,

Fall is approaching quickly so we wanted to make sure everyone is on board with protocol and necessities for trying out for polo this fall.

Please note that there is a window of time to turn in the Athletic Packet: If this is not complete athletes CANNOT participate.

1. READ the Athletic Packet – Link below in item #3

2. TRANSFER STUDENTS – If you are a transfer student from another high school, you have to read the following INFORMATION to make sure you are eligible for the upcoming season. Please contact our Athletic Director, Lou Cesario at lcesario@pleasantonusd.net. to confirm eligibility.

3. TURN IN completed Athletic Signature Forms – Linked here: Athletic-Packet-Master

4. TURN IN a current Physical Examination Form (contained in Athletic Signature Pages – Athletic-Packet-Master) signed by a doctor and stamp included from the doctor’s office on the physical form. An annual examination, or a statement by a medical doctor certifying that the student is physically fit to participate in athletics is required every year. Please note, chiropractor exams are no longer acceptable.

5. BASELINE TEST – It is required to get a Baseline Test (Concussion protocol) completed by our athletic trainer, Diana Hasenpflug, as part of the clearance process. Some sports are not required if they do not have a certain level of contact. The Baseline Testing will be done prior to each season by Diana on campus. We have this scheduled for Monday, August 7th at noon in the library. Incoming freshmen, new players that have not participated in another sport last year and returning juniors will need to have this done. Questions, please ask.

The 5th Annual Pass to Play event will be hosted by Muir Orthopedic Specialists on Sat July 29th from 9:00-3:00 at 2625 Shadelands Dr. in Walnut Creek. The event will consist of an orthopedic, neurocognitive and cardiac pre participation screen. The cardiac portion will have an EKG, review of personal and family history with a physician and an echocardiogram if needed all on the same day. This enables the physicians to rule out any abnormalities which may not be seen from routine exams.

Athletes can sign up for one or all aspects for the same price of $30. All proceeds will be donated back to the athlete’s Athletic Department. Over the past few years we have donated over $14,000 back to local schools. We understand many schools have their own events but just want to be an additional resource for your program and your athletes. 6. SPORTS REGISTRATION – Each student participating in athletics is asked to financially contribute to the operation costs of the Amador Valley athletic programs (varies depending on the sport) at the Student Activities office.
ATHLETICS REGISTRATION FEES for the 2017 Water Polo program is $225.

The money contributed with your registration fee will be used for Coaches stipends, etc.
Donations amounts are updated prior to each season

7. CLEAR any debts or book bills you may have with the school at the Activities Office.

8. CHECK that you are academically eligible to play a sport. Grades from your most recent grading period must reflect a 2.0 G.P.A. with no “F’s.” Grades will continue to be monitored throughout the season by the Athletic Director.

9. ATTEND the mandatory sports meeting (check calendar for dates for specific seasons). Here you will find out more about ASB cards and perks! Mandatory FALL Athletic Parent Meeting- TBA

10. If you are interested in volunteering and have not yet filled out Volunteer Clearance Form, please do so. (Attached)

11. Driver clearance forms need to be turned into Linda Pipe. This is a must- We rely on parent drivers as we do not take buses. Please visit the Amador Athletic Boosters Page for this form.

DO NOT GIVE THESE ITEMS TO THE COACH. ALL ITEMS SHOULD BE RETURNED TO THE STUDENT ACTIVITIES OFFICE WINDOW.
When all of the above have been completed you may begin submitting your Fall Sports Packets to the Activities Desk in the Amador Office beginning on August 1, 2017. The office hours are 7am-4pm. We strongly urge those players that do not need to be screened for concussions to submit their documents on this date. Upon approval of your packet by Activities your athlete will be provided with clearance to participate in tryouts for your sport. A clearance receipt will be issued verifying the completion of the process. Fall Sports will begin the week of 8/14/2017.

*Conditioning Week (Hella Awesome Week!)
Monday, August 7th-Thursday, August 10th 7:30-9:30am and 4-6pm, Friday, August 11th morning practice only.

*Team Retreat – August 11th & 12th more information to follow.
